This is the name of a document used by the Trustee in a chapter 13 case to inform the debtor and attorney of the name and address of those creditors that have filed claims in the case.  The notice also includes the amount the creditor says is owed and the classification of whether the claim is filed as a Secured claim, or as an Unsecured Priority claim, or as a General Unsecured claim.  The notice filed by the Trustee in the Southern District of Illinois includes a classification for any creditor that did not file a claim.  These creditors are listed on the form and classified as Not Filed.
